    Carelon Is Moving Provider Payments from PaySpan to Zelis: What Maryland Behavioral Health Providers Need to Do

    If you receive Medicaid payments from Carelon Behavioral Health of Maryland, your payment infrastructure is changing. Carelon is transitioning provider payment management from PaySpan to Zelis -- and if you have not already taken action to set up your Zelis account, you need to do so now.

    This is not a billing code change or a documentation requirement. It is a change to the actual mechanics of how money flows from Carelon to your practice. Getting it wrong means delayed payments, missed remittances, and the administrative headache of reconciling payments you cannot match to claims. Getting it right is a relatively straightforward one-time setup -- but it does require action on your part.

    What Is Changing and Why It Matters

    PaySpan has been Carelon's payment distribution platform for Maryland Medicaid behavioral health providers. Zelis is replacing it as the payment management vendor. Both platforms serve the same basic function -- they receive payment instructions from Carelon and distribute funds to providers either by electronic funds transfer (EFT/ACH) or check, along with the accompanying 835 electronic remittance advice (ERA) or paper explanation of payment (EOP).

    The practical implications for your practice depend on how you currently receive payments:

    If you receive EFT payments through PaySpan, your banking information is not automatically transferred to Zelis. You need to register with Zelis and set up your EFT preferences -- otherwise payments that were going electronically may default to paper check, creating a lag in your cash flow and a manual reconciliation burden.

    If you receive paper checks, the transition may change where checks originate from and how they are tracked. Registering with Zelis still gives you access to the remittance portal and the option to convert to EFT, which most practices should do if they have not already.

    If you use ERA/835 files for automated posting in your billing system, confirm with your billing software vendor that ERA files from Zelis will be delivered in a compatible format and through your current delivery channel. ERA file specifications can differ between payment vendors, and an incompatible file format can break automated posting workflows.

    The Delayed Timeline

    Carelon originally announced the PaySpan to Zelis transition with an effective date of June 12, 2026. That date has been delayed -- Carelon's current communications note the transition is now targeted for August 2026. As of this article, the exact effective date within August has not been publicly specified.

    The delay actually works in providers' favor -- it gives more time to complete the setup. But "delayed to August" means it is imminent, and every week of inaction is a week closer to the transition without your account configured.

    What You Need to Do

    Step 1: Register with Zelis

    If you have not already registered, go to the Zelis provider portal and create your account. You will need your NPI, tax identification number, and the banking information you want to use for EFT. Carelon has published a transition guide -- the most current version is available via a link on Carelon's provider communications page at maryland.carelonbh.com.

    Step 2: Set Up or Confirm EFT Preferences

    During registration, configure your EFT preferences with your current banking information. If you want to change the bank account receiving Medicaid payments, this is the opportunity to do it. Confirm the account and routing numbers carefully -- payment errors caused by incorrect banking information can take weeks to resolve.

    Step 3: Verify ERA Delivery

    If your practice uses electronic remittance advice (835 files) for automated payment posting, contact your billing software vendor or clearinghouse to confirm that ERA files from Zelis will route correctly to your system. Some billing platforms have existing integrations with Zelis; others may require a new enrollment or configuration change.

    Step 4: Update Your Internal Remittance Workflow

    Once the transition happens, remittance documentation -- EOP statements, payment reconciliation records -- will come from Zelis, not PaySpan. Update any internal processes that reference PaySpan login credentials, portal URLs, or reporting workflows. Staff who manage payment posting and reconciliation should know where to find remittance information after the transition.

    Step 5: Monitor the First Post-Transition Payment Cycle

    After the transition goes live, verify that the first payment cycle under Zelis posts correctly: the payment amount matches your expected reimbursement, the ERA maps correctly to your outstanding claims, and the funds land in the correct bank account. Catching any discrepancy in the first cycle is far easier than untangling multiple payment cycles retroactively.

    What Happens If You Do Not Act

    Providers who do not register with Zelis before the transition effective date may experience:

    Payment delays. If Carelon cannot process your payment through Zelis because your account is not set up, payment distribution may be interrupted until the issue is resolved.

    Default to paper check. Without EFT enrollment, payments that were previously electronic may default to paper check -- slower, harder to reconcile, and more prone to being lost or delayed in the mail.

    Remittance visibility gaps. If you relied on the PaySpan portal to access your remittance advice and do not set up Zelis access, you will lose visibility into payment details until your account is configured.

    None of these are catastrophic, but all of them create unnecessary administrative work at a time when billing teams are already stretched. The setup takes less time than resolving the downstream problems.

    A Note on the Broader Payment Infrastructure

    The PaySpan to Zelis transition is an operational change, not a policy change. Your reimbursement rates, authorization requirements, and billing rules are not affected. But it is a reminder that payment infrastructure -- the mechanics of how Medicaid money actually moves -- is worth understanding and monitoring.

    For Maryland behavioral health providers, Carelon is the single largest payer in most practices' revenue mix. Any change to how Carelon distributes payments deserves prompt attention from billing staff, even when the change is routine.
